If you are a member of the Settlement Class and have not excluded yourself from the settlement, then, by properly filling out, signing and returning a Claim Form with any required documentation, you may be eligible to receive a settlement benefit. Submission of this Claim Form, however, does not guarantee that you will receive a benefit.
